Non Alcoholic Fatty Liver Disease

Fatty liver or NAFLD is excessive accumulation of lipid in hepatocytes.

Nonalcoholic fatty liver disease (NAFLD) includes simple fatty infiltration (a benign condition called fatty liver), whereas nonalcoholic steatohepatitis (NASH) is defined as the presence of fat leading to lipotoxicity and inflammatory damage to hepatocytes. Histologically, NASH is difficult to distinguish from alcoholic hepatitis. Thus to diagnosis NASH, underlying alcohol use must be ruled out. Differentiating simple steatosis from NASH can be difficult without a liver biopsy, and elevated liver enzymes are not a sensitive predictor for identifying NASH.

The presence of metabolic syndrome (obesity, dyslipidemia, hypertension, and glucose intolerance) increases the likelihood that a patient has NASH rather than simple steatosis. Pathogenesis is poorly understood but seems to be linked to insulin resistance (eg, as in obesity or metabolic syndrome). Most patients are asymptomatic. Although noninvasive diagnostic tests are usually sufficient, liver biopsy remains the gold standard. Treatment includes elimination of causes and risk factors; new therapies are rapidly emerging but still in the clinical trial phase. (1)

What I learned from Functional Medicine and Eastern Medicine…

Eastern Medicine hadn’t recognized NAFLD as a problem or condition, probably because the response to high triglycerides would be to counsel on diet, and prescribe blood sugar and lipid metabolizing herbs.  Barberry and Milk Thistle in this study. (3)

But since being enlightened to this incurable condition, Eastern Medicine has Registered NINE Medicines as safe and effective use for NAFLD, including Milk Thistle, all by itself. (4)

The other 8 are herbal blends, most frequently including Hawthorne, Chinese Sage, Japanese Knotweed, Turmeric, Licorice Root, Bupleurum, and Barberry. (5)

Turmeric works in humans. (6)

Bupleurum works in animal models, and with their long history of observing that it is helpful and safe, they don’t have to wait for large scale human trials to approve it for this new condition.  (8)

The dietary counsel, as I understand, would be to eat like an adult.

That means whole foods most of the time, and treats limited to <20% of your calories.  You know, the 80/20 rule.  You don’t have to count.  Just be honest with yourself.

When choosing whole foods, pick what you are most drawn to.  You only crave sugar, which is addictive, so you can pick the whole foods you want without worrying about addictions.

And…you need to coordinate carbs with activity levels.  If you eat lots of carbs and aren’t very active, you will store fat.

Science-y types labeled that the “intuitive diet” and it has shown to improve physical and psychological health as an approach to diet. (14). I suppose it depends on how intuitive you are!?

Eat like an adult, take your medicine for Liver Health morning and night, and do a couple minutes of acupressure when you take your medicine.  Have a daily ritual of moving your body.  While Yoga is a tradition more from India, Calisthenics was the preferred movement therapy of ancient eastern wisdom.

Fish Oil: Scientific Evidence

Randomized Clinical Trials (RCTs) are considered the highest form of scientific evidence.  A group of people with the same condition are divided into two groups, and randomly assigned to receive the medicine, ie omega-3s, or a placebo.  Neither the patient nor the doctors know which group they are in until the results are assessed.

Pediatrics Oct 2020Results: Children randomly assigned to Essential Fatty Acids (EFAs) had a decreased risk of scoring at-risk for Autism Spectrum Disorder on the Pervasive Developmental Disorders Screening Test. (1)

Nutrients Aug 2020Results:  Our findings demonstrate that dietary intervention with high-dose of omega-3 PUFA during non-surgical therapy may have potential benefits in the management of periodontitis. (2)

Nutrients Aug 2020Results: Whey protein and fish oils are a safe, well-tolerated, and effective complement to low-intensity, home-based resistance exercise and improves lean mass, strength, and overall muscle quality in old age. (3)

BMJ Jul 2020Conclusions: Childhood undernutrition is associated with long term impairment in cognition. Contrary to current understanding, supplementary feeding for 23 weeks could improve executive function, brain health, and nutritional status in vulnerable young children. (4)

This study is out of Tufts University School of Medicine, considered one of the best medical research institutions in the world, and published in the prestigious British Medical Journal.  They are not sure if protein, plant polyphenols, omega 3’s, and vitamins/minerals affect health and brain function.  They found it did.  Groundbreaking.

Free Radic Biol Med Aug 2020Results:  Although the inflammatory resolution improved by EPA and DHA could contribute to the recovery of patients infected with SARS-CoV-2, Omega-3 fatty acids supplementation cannot be recommended before randomized and controlled trials are carried out. (5)

Is this code?  Like they want to tell you what could help, but are legally unable to because big pharma has a patent on “medicine” so they tell us this way?

Am J Clin Nutr Jul 2020Conclusions: Fish oils improved cognitive function, especially attention and cognitive flexibility, and reduced socioemotional problems in 8-9 year old children, and the higher the dose, the greater the improvement.  (6)

Diabetes Res Clin Pract Apr 2020.  Results: NRF2 gene expression was significantly increased in Fish oil-supplemented subjects, compared with the placebo group. (7)

That’s the elusive “anti-aging” gene, and now we know, Omega-3s increase health, and health increases graceful aging.

Nutr Metab Cardiovasc Dis Apr 2020.  Conclusion: Regular supplementation with fish oil improved biomarkers of cardiovascular and cerebrovascular function. (8)

PLoS One Feb 2020Conclusion: Omega-3 fatty acids could help ameliorate proteinuria among type 2 Diabetes.  (9). Protein in the urine means kidney damage, so fish oils may reverse the damage?

BJOG Jul 2020Conclusions: Women with single pregnancies and low total omega-3 PUFA status early in pregnancy have an increased risk of early preterm birth.  (10)

Neurologia Dec 2019.  Conclusion: An intake of EPA fatty acid and DHA fatty acid (Fish oil) for 8 weeks is associated with less marked impulsive behaviour in children with ADHD. (11)

Nutrients Oct 2019Results: High-dose Omega 3 oils significantly slowed the progression of atherosclerosis.  (12).  AKA hardening of the arteries, which is a primary risk for heart attack and stroke.

J Neurol Neurosurg Psychiatry Jan 2020.  Results:  Omega-3 oils have significant antidepressant effects for major depressive disorder (MDD). (13)

J Am Heart Assoc Oct 2019. Conclusions: Marine omega-3 supplementation lowers risk for myocardial infarction, Coronary Heart Disease (CHD) death, total CHD, CardioVascular Disease (CVD) death, and total CVD. (14)

Nutr Rev Apr 2020Results: Omega-3 supplementation improves sensory loss and reduces the incidence of neuropathy. (15)

Diabetes Metabol Syndr Jul 2019:  Results: omega-3s have beneficial effects on lipid profile of overweight patients with Metabolic Syndrome, significantly reducing the serum levels of Triglycerides and LDL cholesterol.  (16)

Fenugreek: Scientific Evidence

Cochrane Database Syst Rev May 2020Conclusion:  The trials performed on humans are not of sufficient quality to say with scientific certainty, but Fenugreek has been used for 3000 years to increase a mothers milk supply. (1). And it’s 100% safe to try! (1.1)

Phytother Res Sep 2020Conclusion: Pooled data of clinical trials indicated a significant impact of fenugreek supplementation on lowering Total Cholesterol, low-density lipoprotein (LDL), and Triglycerides and increasing the high-density lipoprotein (HDL) level compared with the control group. (2) (2.1). It also increases insulin in Diabetics so you need less medication.  (2.2)

Fenugreek also lowered HgbA1c, the current measure of diabetes (>6.5) and metabolic syndrome (5.7-6.4) somewhat better than a prescription pharmaceutical.  Fenugreek lowered this number .5, and the prescription medicine lowered it .35.  Another study found it also increases insulin sensitivity which is the core problem in diabetes! Fenugreek works with Type 1 Diabetes too! (2.4)

Complement Ther Med Mar 2020.  Results: Compared to the control group, Fenugreek Seed consumption resulted in a significant decrease in fasting plasma glucose, as well as a significant change in serum alanine aminotransferase (ALT), alkaline phosphatase (ALP), and aspartate aminotransferase (AST) (Liver Enzymes), systolic blood pressure (SBP) (p = 0.001), and a significant increase in creatinine (Cr) (p = 0.001) and decrease in estimated glomerular filtration rate (eGFR) (Kidney Function) in the control group.  (3). The doctors in Iran already knew that in 2014. (3.1)

Fenugreek lowered blood sugar and improved Liver and Kidney function.

Phytotherapy Res Mar 2020Results:  Fenugreek effectively reduced appetite in patients with obesity and/or metabolic syndrome. (4)

J Complement Integr Med Feb 2019:  It can be used topically, on surgical wounds…Results: The pain score was significantly reduced in Fenugreek group in comparison with the placebo group.  Morphine consumption and diclofenc (pain med) suppository demand were significantly decreased in Fenugreek group.  (5)

Climacteric Sep 2018Results: Fenugreek significantly improved sexual function in menopausal women.  (6). And significantly reduced hot flashes. (6.1). It increases estrogen levels 120% on average. (6.2)

Mol Food Nutr Res Jun 2017.  Fenugreek is considered as one of the oldest medicinal plants and its health-promoting effects have been cited in Ayurveda and traditional Chinese medicine.  Extensive preclinical and clinical research have outlined the pharmaceutical uses of fenugreek as antidiabetic, antihyperlipidemic, antiobesity, anticancer, anti-inflammatory, antioxidant, antifungal, antibacterial, galactogogue (milk production) and for miscellaneous pharmacological effects, including improving women’s health. (7)

Aging Male Jun 2016Results:  There was a significant decrease in Aging Male Symptoms score over time and between the Fenugreek and placebo groups. Sexual function improved, including number of morning erections and frequency of sexual activity. Both total serum testosterone and free testosterone increased compared to placebo after 12 weeks of active treatment. Fenugreek seed extract is a safe and effective treatment for reducing symptoms of possible androgen deficiency, improves sexual function and increases serum testosterone in healthy middle-aged and older men.  (8). Fenugreek also increased quality of life and sexual health measures in healthy males. (8.1)

Int J Med Sci Oct 2015Results: Fenugreek caused significant decrease in both ovarian volume and the number of ovarian cysts in women with poly-cystic ovary syndrome (PCOS).  (9)

Euro J Nutr Oct 2016Conclusions: These results suggest that replacing 10 % of refined wheat flour with fenugreek seed powder significantly reduces the glycemic response and the GI of buns and flatbreads. Thus, fenugreek powder may be a useful functional ingredient to reduce postprandial glycemia. (10)

Phytotherapy Res Aug 2015.  The results indicate that this extract of Fenugreek is a useful treatment for increasing sexual arousal and desire in women. (11)

Phytother Res Feb 2014Results: Fenugreek and L-Dopa significantly decreased symptom progression in Parkinson’s disease vs L-Dopa and a placebo.  The fenugreek group had almost zero symptom progression while the L-Dopa alone increased symptoms by 13.36% over 6 months.  (12)

It should be criminal for a doctor to neglect published research and not use this with their Parkinson’s patients.  Or at least advise it’s use from a trusted source.  Like a Functional Medicine doctor!
